The NHS Health Check is a national programme for adults in England aged 40-74. It's designed to spot early signs of stroke, kidney disease, heart disease, type 2 diabetes or dementia. If you are in the age range and don’t have a pre-existing condition, you can expect your GP practice to contact you and invite you for your free NHS Health Check every five years.

If your practice is offering NHS Health Checks, you can request one if:
- you're aged 40-74
- you have not had an NHS Health Check in the last 5 years
- you do not already have a diagnosed long-term health condition
As part of your NHS Health Check, your healthcare professional will discuss your results with you. You’ll be given advice to help you lower your risk of a stroke, kidney disease, heart disease, diabetes or dementia, and improve your health.
If your GP isn't currently offering NHS Health Checks, but you have concerns about your health, you should ask for an appointment with your GP anyway.

Post NHS Health Checks lifestyle intervention programme
Following a NHS Health Check, those who meet the eligibility criteria can be referred to the Post NHS Health Checks Lifestyle Intervention Programme. This 12-week programme introduces individuals to the benefits of physical activity in order to help reduce risk factors for chronic disease and manage existing medical conditions.
The programme provides local residents with a high quality service and affordable physical activity options at ‘BETTER’ gyms within the London Borough of Barnet, along with a 10 week cooking course with nutritional advice.
After referral from a doctor or other health professional a member of the ‘Healthwise’ team will contact you to arrange an appointment. You will then be assessed and a suitable, safe and personalised activity plan will then be designed, taking into account your current physical activity levels and any special requirements that you may have. A dedicated team of exercise professionals are on hand, along with a Health Trainer to offer information and support with physical activity, diet and healthy lifestyles, with your progress monitored and the activity programme adjusted accordingly.
